We operate as a permanent subcontractor inside payer ecosystems, not as a public clinic. Our work centers BIPOC and underserved communities while maintaining a high standard of clinical, ethical, and operational excellence.

Clinical Roles
Psychiatric Mental Health Nurse Practitioner (PMHNP)
Type: 1099 · Location: Remote (California) · Start: January 2026 and forward
SacredHLI is building a small, stable core of Psychiatric Mental Health Nurse Practitioners who want long-term, fully remote work with a predictable caseload and a mission-aligned, trauma-informed culture. You will serve commercially insured members referred and authorized through our payer partners (e.g., Anthem, Aetna, Optum/UnitedHealthcare, Cigna, PPO plans).
Key Responsibilities
- Provide psychiatric diagnostic evaluations and follow-up medication management via secure telehealth.
- Maintain a steady caseload of authorized members, focusing on mood, anxiety, trauma-related, and co-occurring conditions.
- Complete documentation, treatment plans, and follow-up notes to commercial payer standards within required timelines.
- Collaborate with SacredHLI leadership around safety, risk, and stepped-care decisions when higher levels of care are indicated.
- Participate in internal quality-improvement, outcomes tracking, and audit-readiness activities as needed.
- Uphold SacredHLI’s values of dignity, equity, and culturally responsive care for BIPOC and vulnerable communities.
Minimum Requirements
- Active, unrestricted CA NP license with PMHNP certification.
- DEA registration in good standing (or eligible to obtain promptly).
- Comfort with a fully remote, telehealth-only model (home-based practice).
- Commitment to a minimum of 32 billable clinical hours per week once fully live on panels.
- Reliable internet, private workspace, and strong professional boundaries in a telehealth environment.
- Alignment with SacredHLI’s mission and commitment to trauma-informed, culturally aware care.
Compensation (1099 – Structured Monthly)
Compensation is a structured monthly contractor payment, based on maintaining a full caseload of 32 billable clinical hours per week and satisfactory performance:
- Months 1–3: $7,800 per month
- Months 4–6: $8,200 per month
- Months 7–12: $8,600 per month
Year 2 and beyond: eligible for performance-based reviews and potential rate adjustments every
6 months based on reliability, documentation quality, caseload stability, and alignment with SacredHLI standards.
All amounts above assume a full 32-hour billable workload; in periods of reduced hours, compensation may be prorated to match actual clinical time worked.
Psychiatric Medical Doctor (MD/DO) — Telepsychiatry
Type: 1099 · Location: Remote (California) · Openings: Ongoing statewide pool
SacredHLI is building a small, high-trust pool of board-certified or board-eligible psychiatrists to support diagnostic evaluations, complex case consultation, and medication management for commercially insured members. You will work within a payer-authorized telehealth model with strong administrative, billing, and A/R support behind you. High-performing psychiatrists may be considered for future Group or Regional Clinical Director roles as SacredHLI scales across counties.
Key Responsibilities
- Provide psychiatric diagnostic evaluations and ongoing medication management via secure telehealth.
- Manage more complex or higher-risk cases and collaborate with PMHNPs on stepped-care and consultation.
- Ensure documentation meets commercial payer expectations for clarity, medical necessity, and timeliness.
- Participate in quality and risk discussions when requested by SacredHLI leadership.
- Support a trauma-informed, culturally respectful clinical environment that aligns with SacredHLI’s mission.
Minimum Requirements
- MD/DO licensed in California with Psychiatry specialization.
- DEA in good standing.
- Prior telepsychiatry experience strongly preferred.
- Comfort working within a payer-driven, authorization-based model.
Compensation (1099)
Compensation for psychiatrists is structured as a competitive 1099 contractor arrangement (monthly or per-session), calibrated to experience, caseload, and payer mix. Specific terms are discussed during the interview and offer process.
Clinical Leadership Roles
Group Clinical Director (Statewide)
Type: 1099 · Location: Remote (California)
The Group Clinical Director serves as the statewide clinical leader for SacredHLI’s telepsychiatry program. This role provides clinical oversight, supports high-performing MD/DOs and PMHNPs, and serves as a key point of contact with payers on clinical matters. As SacredHLI grows across multiple counties, this position anchors quality, risk management, and consistency of care. High-performing SacredHLI clinicians may be promoted into this role as part of our internal leadership pathway.
Key Responsibilities
- Provide statewide clinical leadership and guidance to SacredHLI psychiatrists and PMHNPs.
- Set and maintain standards for documentation, medical necessity, and clinical workflows.
- Support complex case consultations and escalation pathways for safety and higher levels of care.
- Collaborate with Billing, Compliance, and Program Management on audit readiness and payer expectations.
- Participate in payer-facing discussions on utilization, outcomes, and quality improvement.
- Mentor emerging clinical leaders and help identify internal candidates for regional leadership roles.
Minimum Requirements
- MD/DO in Psychiatry or senior PMHNP with substantial leadership experience.
- Extensive telehealth and managed-care experience.
- Demonstrated strength in risk management, documentation standards, and clinical systems-building.

County / Regional Clinical Director (Telepsychiatry)

As SacredHLI expands into multiple counties, County / Regional Clinical Directors will provide localised clinical leadership while still operating in a fully remote model. This role focuses on supporting NPs and psychiatrists serving members in a defined region, ensuring consistent quality, documentation, and escalation practices. These positions are ideal for experienced SacredHLI clinicians seeking to grow into leadership, or external candidates with strong multi-site telehealth experience.
Key Responsibilities
- Provide regional clinical support and case consultation to SacredHLI psychiatrists and PMHNPs.
- Monitor documentation quality and timeliness for members within the assigned county or region.
- Support escalations related to risk, safety, or complex clinical presentations.
- Coordinate closely with the Group Clinical Director, Program Manager, Billing, and Compliance.

Operational & Administrative Roles
Program Manager — Telehealth Operations
Type: 1099 · Location: Remote (California)
The Program Manager supports day-to-day telehealth operations, provider coordination, and KPI tracking across SacredHLI’s payer-based psychiatry program. This role is ideal for someone who understands behavioral health workflows and enjoys building stable systems quietly behind the scenes.
Key Responsibilities
- Coordinate NP and psychiatrist schedules alongside billing and authorization timelines.
- Monitor key operational metrics (show rates, documentation timeliness, capacity utilization).
- Support onboarding and orientation of new clinicians.
- Work closely with Billing & Claims Manager to ensure encounters are ready for billing.
- Escalate operational risks, bottlenecks, or patterns to leadership early.

Billing & Claims Manager

The Billing & Claims Manager owns the financial lifeline of SacredHLI: clean claims, denial management, payer portal oversight, and coordination with both our billing partner and A/R factoring partner. This role is critical to ensuring predictable cashflow and audit-ready financial records.
Key Responsibilities
- Oversee claims submission workflows and monitor payer rejections/denials.
- Work with SacredHLI’s external billing partner (e.g., Billing Paradise) to resolve issues quickly.
- Coordinate batches and reporting needed for A/R factoring (e.g., Plex Capital).
- Maintain payer portal access, updates, and routing of key correspondence.
- Provide regular reporting to leadership on revenue cycle KPIs.

Compliance Manager

The Compliance Manager helps SacredHLI stay ahead of payer requirements, HIPAA, internal policy, and audit readiness. This role focuses on practical, right-sized compliance that supports a lean, remote operation without unnecessary bureaucracy.
